To arrange for enrollment in a Parent Information Program, fill in all the blanks on the following form. When finished entering information on the form, you MUST click the button called “Submit” just once in order for your request to be sent. A few seconds later you should see a confirmation screen.

Walk-ins are allowed as long as the class size has not been exceeded and you arrive a half hour prior to the start of the class with all required items listed on the “Parent Information” page.
